AmeriCorps FT Paperwork Upload

Hello MTCC AmeriCorps Service Member!

After downloading and filling out hiring paperwork documents from our website, scroll down this page and follow directions to upload. Each document will need to be submitted as a separate scan/document. On the right side of the page under each item to be uploaded are instructions and reminders. PLEASE READ AND FOLLOW THESE INSTRUCTIONS and REMINDERS. When you are ready to upload each document, click the orange "choose file" button on the left and choose the appropriate document to upload from your computer files. All documents must be uploaded before you click the final "submit" button at the bottom of the page. If you do not have all your documents signed and ready to submit, save what you have completed, and come back to finish later. You must submit your paperwork by the date listed in the original email sent to you.

PLEASE DO NOT SEND THESE FORMS VIA EMAIL. Because you are submitting personal and sensitive data, we can only accept these documents through Submittable's secure platform.
